Left heart hemodynamics during angina pectoris induced by atrial pacing.

چکیده
In order to determine the associated hemodynamic events, angina pectoris was induced by an atrial pacing technic. In all patients developing angina pectoris, ischemic changes were noted electrocardiographically. The hemodynamic findings were variable: in some patients no changes were noted; in others, significant increases in cardiac output, left ventricular end-diastolic pressures (LVEDP), and femoral arterial and pulmonary arterial pressures occurred. These changes suggest that, initially, ischemia produces a decrease in myocardial compliance and this may also be associated with enhanced sympathetic nervous system activity. Myocardial failure may subsequently ensue, however. The administration of nitroglycerin, during angina at a fixed rapid heart rate, resulted in a reduction in LVEDP and cardiac work. Chest pain was always relieved following these hemodynamic changes, indicating a dependence upon these changes for the effective-

متن کاملHemodynamic Accompaniments of Angina A
The hemodynamic responses of nine patients with severe coronary artery disease were studied during the precipitation of angina by both supine exercise and increasing rates of atrial pacing. Tension-time index and the first derivative of left ventricular pressure pulse (LV dp/dt) at the onset of angina were significantly higher (P < 0.01) in each patient when angina was induced by exercise than ...
متن کاملSequential Alterations in Myocardial Lactate Metabolism , S - T Segments , and Left Ventricular Function During Angina Induced by Atrial Pacing
Myocardial lactate metabolism was monitored before, during, and after a period of atrial pacing with a continuous automated sampling technic. The electrocardiogram was recorded, and left ventricular function was evaluated by relating left ventricular enddiastolic pressure to left ventricular stroke work. Seventeen of 21 patients with coronary artery disease experienced angina during pacing, and...

متن کاملInvestigation of the physiological basis for increased exercise threshold for angina pectoris after physical conditioning.
Eight patients with coronary heart disease and exertional angina pectoris successfully completed an 11-15 wk program of endurance exercise conditioning. Angina threshold was determined by upright bicycle ergometer exercise and by atrial pacing. The product of heart rate and arterial systolic blood pressure at the exercise angina threshold was higher after conditioning. suggesting that condition...
